Hospice at Home

giving choice to patients

Our Hospice at Home Care service aims to provide you with overnight care when you are within the last four weeks of your life. The professional care you would receive from our nurses will enable you to stay at home to maintain the best quality of life and dignity.

This service started in August 2010 and now receives referrals from all five GP surgeries in Corby. Also, any Lakelands patient who has used our Day Services in the earlier stages of their illness, will also be able to have more intensive nursing care at their end of life. The service is available 365 days a year.

HH Team

Hospice at Home Nurses will work very closely with your GP and District Nurses and share the care that needs to be given over a twenty four hour period. This will allow you choice about where you wish to be cared for and may avoid hospital admission. Currently Cynthia Spencer Hospice provides a Hospice at Home Service for Northamptonshire but our service will complement that service specifically for the people living within the Corby area. Additionally you must be over 18 and be living in Corby. More recently, Hospice at Home has expanded to provide respite care for patients and their families. This service will not be every night but will typically be 2 or 3 nights a week, to allow relatives/carers to catch up on their sleep.


All Lakelands Hospice at Home Nursing staff:

  • Are Registered Nurses with additional training in palliative care.
  • Work in accordance with the Nursing and Midwifery Council (NMC) guidelines.
  • Will provide support for you, your carer and family member’s throughout our intervention.
  • Will always respect the personal property of patient’s and treat it with due care at all times.
  • Maintain Lakelands Health and Safety Policies.
  • Will undertake a risk assessment for every patient and will ensure records are maintained should an accident or incident occur.
     

Lynn Davies our Hospice Home Co-Coordinator said “Patients will be referred to the Hospice at Home service in the last four weeks of life. Referrals will come from district nurses. The service will run from 10pm until 6.00 am liaising with the district nurses during the day, to attend the patient overnight and then handing back to the district nurses the next morning. The service will be operated by a specialist team of qualified bank nurses. In order to maintain continuity for the patient wherever possible the same nurse will attend the patient each night."

Tracy Glen (Lakelands Nurse Manager) says “How people die remains in the memory of those who live on and it is important to get this right. Our service aims to give patients choice and for us to make that choice become a reality”.
